Solved 100% Disk Usage, computer nigh unsuable, BSOD for CHKDSK

What if I told you that the problem appears to have resurfaced? His computer recently just froze at the login screen for Windows, and it almost seemed like we couldn't get past it. Using patience, we eventually logged onto his computer. However, everything is almost just as slow as before. Task manager also shows his disk usage at 100% again, even though it is only using like .2mb/s. He can hardly open any programs, and his explorer has crashed a couple times along with his mouse and keyboard.

Since we already restored Windows and that only proved to be a temporary solution (even with the full reinstall and the upgrade to Windows 10), what do you think the problem is / what could we do to fix it?
