Strangely, the windows card reader/usb safely remove icon reappeared after a start-up. It as usual still recognizes the USB stick by name, when I plugged in a SD card, the what do you want to do with this pop-up appeared and I chose open folder. It opened the folder on the SD card. When I went and clicked on the remove device icon, it did not show the SD card and had no ability to remove and again showed USB Mass storage device. Looked in Hardware Manager and now it showed under USBs, both a composite reader and a USB Mass storage device, with the last indicating an error.
Just for those interested and thinking that they might want to figure it out. Not using it, wouldn't mind, but not necessary. Call it an intellectual exercise for the learned.
    Thanks for any ideas!

    No worries, Glenn
One thing not many people aware that is by default Windows have all USB drive is set to "for quick removal" (you can check it on its properties). I've been doing unplugging any USB device without clicking "safely remove" thing since the first time I have USB drive unless I change the cache setting for the device.
In short, if you are sure it is not in the middle of writing to the device you can just unplug with no guilty at all. Even if the content is being displayed in explorer, when you unplug it the explorer will automatically closed.

Edit: This is the default of all USB drive. Check it out with the explanation given by windows.
