I uninstalled KB2975719 and all seemed fine for a while, but then the problem returned. I have to say its been driving me mad and I've spent hours trying to work this out. I'm no expert, but this file caused the problem - mfmp4srcsnk.dll. I've no idea what it does but after hours searching...
