Windows 8.1 pro Clean install partitioning issue.

Everytime i try to install windows 8.1 the os creates 6 gpt partitions

im installing the os on an unallocated 698GB drive.
i just want a clean install on the hard disk drive with one partition for the system reserved.
instead windows is creating 6 gpt partitions with different file systems as you can see in the photo,which won't allow me
to shrink the c drive and create a new seperate partition for a d drive to put my data in.
This is the second time i install the os
i have an ssd and a hdd im basically installing the OS on an empty unallocated hdd and thats how windows is creating the
partitions for me every time.
Can you guide me on the best way to set up my partitions,so i can have just two partitions a (C) for the os and (D) for data
and the 350 MB system reserved partition all on the hdd.
