Directx error : DXGI_ERROR_DEVICE_REMOVED

I've recently been getting some crashes when I play games. I've tried changing my drivers to beta, old, newest and they still occur. Is this a graphic card problem? The pictures are attached. Thanks alot :D My graphic card is a GTX 560 overclocked 1gb
