I have the exact same problem. I have 3 monitors running. I notice if I leave the computer on for a while and finally add or disconnect my tv desktop making it 4 desktops or reducing it to 3 it happens. Sometimes it happens by opening windows explorer or just getting into a league of legends match. This problem occurs once or twice a day. I have to log out to fix it. I'm running windows 8.1 64 bit, amd 290, logitech g19 keyboard and logitech g600 mouse.

plug it in to the GFX card, i had it setup the same and since i stop using it mine has been fine so far
If you cant do that use a program called DDU (Wagnard Tools) unplug the monitor and do a clean uninstall of the intel and try a different driver
