I am confused. The information from diskpart is different from the Disk Management screen shot on your first post which includes 128 MB unallocated space. Normally, you can only see this partition from diskpart, not from the disk management. Can you double check ?
if different, can you post the screen shot of the disk management again.

Did diskpart again. Attached is the disk management for this moment.

    DISKPART> select disk 0

Disk 0 is now the selected disk.

    DISKPART> list partition

    Partition ### Type Size Offset
    ------------- ---------------- ------- -------
    Partition 1 Primary 1000 MB 1024 KB
    Partition 2 System 260 MB 1001 MB
    Partition 3 Primary 217 GB 1389 MB
    Partition 4 Primary 350 MB 218 GB
    Partition 5 Primary 12 GB 218 GB

OK. Now it's kind of make sense to me. Let us try from disk management:
    1. Right click on the 128 MB unallocated space->New Simple Volume
    2. let Windows assign to any letter->next->next->finish.
    3. Try to enter the recovery mode again.

Yes, created new partition. Restarted PC, entered recovery mode (F11). Got the same error.

Hmmm. Open the Admin command prompt and type: bcdedit /export saveBCD . This will create a backup copy named saveBCD of your current BCD file, put in a zip file then attach it for me. I'd like to take a look and I'll get back to you.

I had created new partition in the prev step with disk manager wizard, so I dont have this 128mb unallocated space anymore (see attached screenshot). Do I have to remove that 128 mb partition in disk manager before running this "create partition msr" command in diskpart that you suggested? Thanks.
