Or maybe the inability to boot the flash drive is due to the Setup changes reverting back to default.

I would agree ,usual cause of this was/is a dead bios battery

Yes, the boot menu access is with the Esc key on this computer. It only shows the Windows Boot Manager. When I try to modify that, it resets itself to default.

On my ASUS K75DE Laptop hitting the Esc key brings up the one time quick boot menu. If I hit that I can select my thumb drive as a boot option for that one boot up. On the next boot up it reverts back to the BIOS default, my hard drive. On some PC's the thumb drive has to be formatted in the Fat file system to install in UEFI mode. My laptop is fine with NTFS though. You should get two boot entries for your thumb drive, one with UEFI in the description and one without, assuming it was created correctly. It was F9 for the factory reset options on my laptop too. What I got was the advanced options to do a reset. Reset Windows 8 I've since done a clean install so it does nothing now.
The hard drive can be shown in the BIOS and even pass the SMART tests but still fail to load the OS. If the file system gets corrupted the OS won't load. Physically the drive is fine, but the data on it is mangled.

Setup shows the correct date/time, so I think that indicates battery is OK. And laptop is only 13 months old.

Just curious, but nowhere in this thread did I see mention of which model this was. I know it's an Asus, but that's it.

BTW here's something odd my Asus Zenbook Prime (UX31A-DH71) ultra book does. Sometimes when the battery is low (not dead), it won't boot unless I plug in the power cord. You'll see the keyboard light up, and the backlight of the screen, but that's it. When I plug in the power cord all is good. I've no idea why it does this sometimes

I'm sure after all this time you've plugged it in, but...

    It's an Asus G46V.

Your symptoms are exactly what this is doing. It is plugged in, but I'll put a voltmeter on the cord to confirm it has correct voltage.

DC voltage is good, and the charging light is on, on the front of computer.

How about if you pull the battery and plug in power? Really good to eliminate this stuff.
