Minimized windows disappearing?

Under Windows 8, when I minimize a window it disappears completely?
Have I somehow changed a setting for the actions of the minimize button?
Task manager show the program running but nothing appears in the taskbar.
All programs are doing this so it must be a Windows 8 setting?

Click on blank area of TaskBar & select Properties. Against Task Bar Buttons option, make sure that 'Never combine' is selected.

Sometimes, the taskbar will start showing two rows, and there will be an arrow on the Tray side. This started in Vista, may still be happening in 8. It's actually caused by the "QuickLaunch" toolbar being in the wrong place. Windows 7 and 8 don't have a quicklaunch bar, they took that out in favour of pinning to the taskbar and Jumplists. But still, you may have a toolbar dragged to a second row on the taskbar. If this is the case, you will see an up or down arrow right next to the Tray items.
