BSOD plugging in USB Modem

Hi all,i have been experiencing BSOD lately when plugging my USB 4G Modem in. This modem worked well before for about 5 months.By using "WinDbg (x64)" i have found the problem to be with the driver "usbccgp.sys" however i believe this is a generic driver and the crash is caused by an underlying driver specific for my USB Modem. I have tried uninstalling and reinstalling the drivers for my USB Modem but still BSOD.Also i have performed a registry clean, sfc /scannow and chkdsk /f.I have attached the log file as created with the posting instructions. Your help is very much appreciated!
