Solved BSOD APC_INDEX_MISMATCH ntoskrnl.exe

I have had an APC_INDEX_MISMATCH BSOD almost daily for the past several days. I've not been able to relate it to any newly-installed software or hardware or to any system changes. I'd be obliged if anyone could help. Thanks very much.

Update: These same BSODs have continued apace, and I have attached additional zip files. Perhaps it should be noted that the CPU is running at 100% almost constantly. The following processes especially seem to be using more CPU resources than they have in the past: explorer.exe, WmiPrvSE.exe and, sometimes, the System process (ntoskrnl.exe).

Second update: I've also attached a system report taken just after a driver-verifier-induced crash. I had to disable the driver verifier after that because it was causing the crashes to occur even more frequently.

Update drivers in red or uninstall the software related to it. If that doesn't do the trick, please temporarily uninstall VBox.

There is some important information that I didn't think to mention before:

At about the same time that I was experiencing the BSODs, I was also having a problem with Start8, which provides a Windows-7-like Start menu for Windows 8. It was running, but not providing the Start menu as it had before. At first, I didn't think to see if there might be any connection between the two issues, but after my initial posts here, I continued to try figure out what might be causing the BSODs. My attention focused on the very high CPU usage that the system was showing: 100% usage almost 100% of the time, with the System process (ntoskrnl.exe) and explorer.exe each taking up anywhere from about 45% to 75% of the CPU. The high usage by the System process (ntoskrnl.exe) was opaque to me, but it seemed that perhaps something that modified the shell might be causing the high usage by explorer.exe. Since Start8 must certainly in some way modify or interact with the shell, and since it was not behaving properly, I decided to see if uninstalling it would make the BSODs go away. It seems to have done so. The high CPU usage and the BSODs ceased immediately upon uninstalling Start8 and have not come back. I'm going to leave Start8 uninstalled another day or so. After reinstallation I'll let you know whether or not the BSODs recur. Considering this, I have not yet implemented your advice above. Thanks very much for your help so far.

Since uninstalling and reinstalling Start8 there have been no more BSODs, so I'm going to guess that the installation somehow got corrupted. Thanks very much.
