BSOD 3-4 times/week, possible issue with Windows Kernel

I built my current system about a month ago. Since then, I've had numerous BSODs, some caused by my graphics card driver. I updated the driver and the BSODs became less frequent. However, I still get them regularly, usually when waking the computer from sleep mode. I've had a couple at random times as well. I've tried to figure out (using WhoCrashed and WinDbg) what was causing these issues but to no avail. I ran Driver Verifier with all the drivers checked and have not gotten any BSODs from it. I have not yet run a Memtest since WhoCrashed said that the BSODs were not likely to be caused by hardware issues.

I noticed that you don't have Secure Boot enabled. If you were having problems with it and changed it, please let us know.
It's not necessary to enable it now. But, should you reinstall Windows at some point in the future, please enable it first.

I mention this because it may happen that (one day) the system won't boot. This can be caused by a program changing your UEFI settings, or an update of the UEFI resetting it to default values.

To test and see if this is the cause, boot into the UEFI and see if the settings have been changed. If uncertain, try with Secure Boot both on and off (and the UEFI on UEFI or Legacy (CSM))

If it still doesn't boot after trying this, then move on to other troubleshooting tools as it's not likely to be due to this.

Out of the last 7 memory dumps there were 7 different BSOD error codes. The differing error codes are usually symptomatic of a lower level problem within the system. They are usually caused by one of these things:
- borked (broken) hardware (several different procedures used to isolate the problem device)
- BIOS issues (check for updates at the motherboard manufacturer's website)
- overclocking/overheating - You'll know if you're overclocking or not. If uncertain we can suggest things to check.
- missing Windows Updates
- compatibility issues (3rd party hardware/drivers) - and older systems
- low-level driver problems
- or even malware (scanned for when we ask for hardware diagnostics from Initial Hardware Diagnostics or Hardware Diagnostics ).

You can get a head start on the hardware diagnostics if you'd like - they are located here: Hardware Diagnostics

Have you enabled Driver Verifier? I ask as there are several Driver Verifier enabled memory dumps (they will, on occasion, happen without Driver Verifier being enabled).

Just on a hunch, I'd suggest trying this first:

Or, you may want to uninstall the nVidia drivers and then install a fresh copy without the Streaming Kernel Service (I'm not familiar with nVidia drivers, so I don't even know if this is possible).

Once you finish these steps (including the hardware diagnostics), post back with your results and we'll move on from there.

What exactly is Secure Boot? I'm pretty sure I never changed the default setting.

I updated my BIOS to the most recent version quite a while ago to try to fix BSOD issues. The update was successful but it didn't seem to make much difference.

I'm not overclocking, and I've checked my temps using Prime95. Everything's safely within normal range.

I have all available Windows updates.

I've used Driver Verifier multiple times. It led me to the same nVidia streaming kernel service, which I disabled (the result was significantly fewer BSODs).

The system is built with all new parts, so there shouldn't be any old hardware issues.

I have two disk drives: a WD Blue for storage and a Samsung SSD. I ran a Lifeguard Diagnostic check from WD on that drive and it didn't come up with any errors. I ran Samsung Magician software which "optimized" the OS and SSD; not sure if that did anything useful.

I ran two passed of Memtest86+ and did not find any errors.

Something worth mentioning: when I was initially installing mobo drivers, there was one that I couldn't install. I just checked to try to figure out which one it was, only to notice that Gigabyte just pushed a bunch of new updates for my mobo (within the last month). I'm going to flash the BIOS again and install some updated drivers and see if it has any effect.

Attached is the dump file from my BSOD today (went four days without one, then had one a few hours ago).

One last thing: looking through all the dump files, these are the process names I pulled out from them. I can't draw many conclusions from such a diverse list, but I thought it couldn't hurt to include it here:

Secure boot is a complicated topic - basically it prevents malware from injecting itself early in the boot process (where antivirus programs can't detect it). Most times it's turned off in order to boot from a CD/DVD/USB drive. It's not a worry right now, but if you can't get into Windows after updating your BIOS/UEFI - that's the first thing to check (the update may turn it on by default, so you may have to turn it off to get back into Windows).

If you continue to get BSOD's w/the nVidia Streaming thing disabled - try running Driver Verifier again to see if it picks up another driver to blame.

In general, process names aren't significant to BSOD analysis. The only time that I pay attention to them is when there's a string of the same one's named in the BSOD's. In that case I have to wonder if there's a link between the .exe and the BSOD's (it's a clue that something is missing from the analysis - and it's our job to find out just what that is).

FYI - BSOD's occur in the kernel space - while most exe's only run in the user space. So, exe's usually can't cause BSOD's directly (but there are exceptions to this) as they don't operate in the kernel space.

Please note that, in this memory dump, the NvStreamKms.sys is still present. If you've updated your video drivers, it may have been re-enabled that way. Regardless of why, it still needs to be disabled/removed.

Make sure that you update everything that Gigabyte has posted. This includes all things from Intel. Don't worry about the Intel SATA F6 driver - that's only for new installations.

I installed everything from Gigabyte, except the Intel SATA Preinstall driver. When I unzipped it, it did not come with a setup.exe. Is there something that I'm missing here? The download link is here: GIGABYTE - Motherboard - Socket 1150 - GA-H87-D3H (rev. 1.x).


I did update my video drivers, so that's probably what re-enabled NvStreamKms.sys. I think I disabled it now. I ran Services.msc and stopped and disabled startup for "NVIDIA Streamer Service," located at "C:\Program Files\NVIDIA Corporation\NvStreamSrv\nvstreamsvc.exe." Is there anything else with this service that I need to disable?

I just ran driver verifier again. It took a couple hours, but I just got a BSOD. Looks like it's iqvw64e.sys, something to do with the Intel Network Adapter Diagnostic Driver. The dump file is attached.

EDIT: Just got another BSOD. Woke the computer from sleep mode, Chrome stopped responding, then BSOD. Dump file is attached.

The SATA preinstall driver is for installation BEFORE you install Windows. You don't need it for an existing Windows installation (the Intel RST has the same drivers built into it)

The Intel network adapter diagnostic driver (iqvw64e.sys) dates from 2012.
Please update the Intel networking drivers (both wired and wireless) from your system's support/download page.
Ensure that you get drivers that are compatible with Win8.1 - NOT Win8

If unable to get the 8.1 drivers there, then try http://downloadcenter.intel.com

Then we'll wait and see if the BSOD's stop......

I updated all the Intel drivers which were out of date, including the LAN. I haven't gotten a BSOD since, but it's only been a little while.

Just now though (and it's happened before as well), my computer crashed/rebooted when waking up from sleep mode. I don't believe there was a BSOD. There is a delay when my monitors turn on, so there could have been one that I didn't see, but there was no dump file created.

Timeline of events: just after 1 pm, I put the computer into sleep mode. At about 2 pm, I attempted to wake it up. It then crashed/rebooted.

When I open up Event Viewer, it shows three errors:
Error at 1:59: The previous system shutdown at 1:08:20 PM on ‎6/‎16/‎2014 was unexpected.
Critical at 1:59: The system has rebooted without cleanly shutting down first. This error could be caused if the system stopped responding, crashed, or lost power unexpectedly.
Error at 1:59: Audit events have been dropped by the transport. 0


EDIT: Just had another BSOD. Again when waking from sleep mode. Dump file is attached.

Yet this seems to me to be a driver problem.
In that case, please run Driver Verifier according to these instructions: Driver Verifier Settings

If the Driver Verifier crashes don't reveal anything, then I have to wonder about a hardware problem.

Been running driver verifier for a little while now. I got two more BSODs, but I don't think they were caused by verifier. The dump files are attached.

It looks like they're pointing to nvlddmkm.sys. Not exactly sure what to do about that.

EDIT: just had my system freeze completely, but not BSOD. Happened right after waking up from sleep. Chrome crashed, then complete freeze. In event viewer, I keep seeing errors saying that the last system shutdown was unexpected. This makes me think it's an issue with the computer going into sleep mode that's then realized when waking back up.

The last memory dump was verifier enabled and pointed to the nVidia drivers.
This means that, most likely, this is a problem with the video (note that I did not specify drivers or hardware - more on that later)

The first step is to:
- download a fresh set of the latest, Win8.1 compatible nVidia drivers from the nVidia website
- uninstall the current nVidia drivers
- install the freshly downloaded nVidia drivers
- monitor for further BSOD's

While waiting to see if it BSOD's, also try these free diagnostics (start with the video tests): Hardware Diagnostics
Also, ensure that the card is firmly seated in the slot, that the card is receiving cool air, that the fan on the card is working, and that the cooler is not clogged with dirt/dust/hairballs/peanut butter and jelly sandwiches/etc :0)

Update:
I uninstalled the nVidia drivers, then reinstalled the latest versions, minus some gaming-specific items that I don't use. I then disabled the streaming service that everyone seems to have problems with.

I uninstalled the Intel graphics and audio drivers, as I have the nVidia for graphics and a Realtek driver for audio. I also disabled the integrated graphics via the processor.

Driver Verifier has been running for just shy of 48 hours now and I haven't had any problems. My system passed the FurMark tests with flying colors. No matter the resolution, the GPU temp never exceeded 67 C (tests were about 20 minutes each).

EDIT: I thought everything was good, but I just had another BSOD (about 50 hours after enabling driver verifier). I think this one might shed some new light onto what the potential problem could be. Dump file is attached.

EDIT 2: Just ran a Malwarebytes scan to rule that out. It found three things, all related to OpenCandy. I quarantined these items.

The Furmark results are good, but there are other hardware components (and other components of the video card) that could be having problems.

If the system passes all of the other diagnostics, then please use this procedure to help isolate the troublesome hardware: Hardware Stripdown Troubleshooting

This is not a Driver Verifier enabled memory dump. Please turn verifier off as it's done it's job and is no longer needed. To do this, open up verifier.exe and select "Delete existing settings", then click on "Finish" and reboot the system for the changes to take effect.

Sorry it took so long for me to get back to you.

Test that I ran: Prime95 (been using this to stress test my CPU and cooling system since I built my system), FurMark, in-Windows test on my WD HDD, in-Windows test on my Samsung SSD, Memtest 86+. Everything passed with no issues.

Just today I ran a Windows check disk on both drives. The SSD (which contains the OS) had some issues that I repaired. I'll see if this fixes the BSOD issues. I also had some issues where a certain DLL was not found after waking up from sleep mode and I couldn't launch an application (exception code 0xc0000005). This corrected itself after restart. I'm assuming it has to be something with the file system, so I'm hoping that repairing the SSD will fix the problems.

I'm hesitant to do a hardware stripdown test since it usually takes a few days for a BSOD to occur. I'm going to see if the problem is now fixed. I'll reply again in a few days.

EDIT: No luck. Attached is the most recent dump file. This time it happened while the computer was on, not when waking up from sleep. All these issues are having me consider reinstalling Windows to see if that could be the issue. I could also possibly take out the video card for a few days and run my monitors off of the on-board graphics to check if that is the issue.

Sorry for the delay in responding.

This one blames the NTFS file system.
I've done a bit of research on this and am finding that there may be issues with mixed SSD and HDD systems
The fix usually is to reset the Intel Smart Response Technology - but can't find how to do this.

Try opening the Intel Rapid Storage Technology UI (I found it by going the the Start screen and typing "Intel" (without the quotes))
Then click on the Performance tab
Then, in the left sidebar there should be an option for Smart Response Technology
Reset the settings in that (I don't have it on my system as I only have one SSD)

Also, while you're there, please let us know what versions of the
Go to Help in the IRT interface, then click on System Report
Click on the "Save" button in the lower left corner and save it as a text file
Then please upload it with your next report.

As an aside, I wonder if removing the HDD would stop the BSOD's.
If you can try this, it may be revealing.
Unfortunately, if it does fix it - it means that you wouldn't be able to access extra storage :(

Reinstalling Windows might work. Should you choose to do this, don't install anything extra until you're sure you're not getting BSOD's
If possible, use a retail disk to reinstall Windows as it'll only have Windows approved drivers on it.
Then check Device Manager to ensure that there a no problems there. If you have problem devices you'll have to get the latest updates from the device manufacturer's website.

When I look at the Performance tab in the IRT UI, there is no left sidebar. All that I see is the option to enable/disable Link Power Management (I've always had it enabled). Disabling it and rebooting does not bring up any additional options in that tab.

I've also looked up the Smart Response Technology and my understanding is that the SATA controller needs to be set to RAID (I could be wrong in my interpretation). I've previously tried going into the BIOS and changing the configuration from AHCI to RAID, but this just resulted in my system not booting until I changed it back.

When I built the system, I only plugged in the SSD while installing Windows and configuring the system. Once that was finished, I plugged in the HDD and changed the location of the Documents, Pictures, etc. folders to be on the HDD rather than the SSD.

I had an interesting event happen today. I tried to wake the computer from sleep mode (by pressing a key on the keyboard). The power and HDD lights turned on and the fans started spinning, but the monitors remained black. After about 2-3 seconds, the lights and fans turned off. Pressing another key on the keyboard did nothing. When I moved the mouse, the system responded (lights and fans on), the motherboard splash-screen appeared, and the system started normally (as if from a power off state rather than from a sleep state). There was no BSOD/bugcheck. I've had this happen a few times.


Another interesting thing that might be worth mentioning is that the delay between when I wake up the system and when the monitors turn on varies by a factor of at least 2 (i.e., sometimes it takes twice as long as other times).

I've noted issues with sleep/hibernate - and do find that they will crash without letting you know (resulting in a clean boot). The only clue will be that your open programs won't show when the system starts back up (if it's crashed).

If the system is using AHCI and not RAID, then the Smart Response probably won't be active. I checked this with several laptops at work and didn't find one that used the RAID. I did check an Alienware desktop with 2 hard drives and didn't find it there either (I couldn't access the BIOS at the time as someone else was working on it - and I didn't notice that the mode was in the SystemReport).

In late May Intel came out with the v13.1 series of the Intel Rapid Storage Technology drivers. You are using the v13.0 series - which is the latest that Gigabyte offers. I was only able to find RAID versions of the 13.1 series at Intel, so updating may not be an option here. You may want to search yourself - here's the link: http://downloadcenter.intel.com

Some people report success by disabling Fast Startup/Fast Boot. Have you tried that?

I've had Fast Startup disabled for a while now (apparently it's necessary when using WOL with Teamviewer). I looked for a non-RAID IRT download and couldn't find one either. I'm pretty busy for the next week or so, but after that I'll really get going on taking components out and seeing if it's hardware-related. Is there anything else software-related that I can test/check?
