    Ok, great to hear that. Open Device Manager and on the USB entry that has an exclamation point, right click it and choose Update Driver Software.

    Let it search automatically to install the driver and it will probably install the default Windows 8 USB 3.0 driver.

    I have attached a file showing the issue. USBxHCI has a yellow triangle but if I am understanding well I am also missing the driver for the hub: Intel(R)_USB_3.0_eXtensible_Host_Controller driver for win 8.1. Looking in Device Manager under 'Universal Serial Bus Controllers', Its no were to be seen. Nevertheless, I went on and right clicked on the yellow question mark then on update driver on the net and it said that I had the best driver installed (see screenshot 2). I made a screenshot of the error Windows is reporting as far not being able to load the driver (see screenshot 3). I have to admit that this is confusing. I tried to go on the intel website and no driver for Win 8.1. I read on some blogs that Windows is supposed to take care of that driver. So I uninstall the driver (USB xHCI) and restarted my rig. The driver could not be reinstalled with the exact same error message shown in screenshot 3. confused.com Thanks you for your time and kindness MasterChief. Without your help I would be completely lost at sea.

    I don't think that will actually fix it, but it's just for good measure.

    We have to be kind of careful with what we do, because we don't want the BSOD issue to crop up again, so we are left back at square one.

    I gotta be honest with ya - I pulled a bit of a bonehead mistake and it's the first time I ever did this after fixing literally 10s of thousands of machines online with BSODs heh. That USBXHCI.sys we deleted was actually a Windows operating system file (in error, I thought it was an Intel 3rd party driver.)

    So, this might put it back:

    Open an elevated command prompt and run this command:

    sfc /scannow

    But keep in mind, it might actually cause the original issue all over again.

    If it does, then in safe mode, rename it to USBXHCI.sys.bak.

    Not to worry - tell me results of all this and we will get it straightened out one way or another, for sure.

    If it did cause the issue again, attach latest crash dump so I can see the state of the machine as it stands.

    Beauty - All is back to normal.Unit is stable again.USB 3 is up and running - All my USB ports are recognized and working.Here it is:Step 1: As you suggested: sfc /scannowStep 2: Restarted the unit and Windows did the rest. It downloaded all the drivers and all the usb ports came alive. See screenshot attached. MasterChief: Thank you for your time and effort. I could not have done it without your help. All the best.
