Tile missing on every logon

I right click on the .exe file in the file manager and select Pin to Start Menu. The Tile appears and works. However, the next time I log on that tile has disappeared. Any ideas on what's happening?

Word.exe from the desktop. It pins to the START screen until I reboot.

Maybe your actions are creating some sort of conflict because it is already available for pinning from the Start screen itself? Try this: Go to the Start screen, right-click to bring up the "all apps" icon (lower right), click that to display all the apps icons, look for Word there, right click it and pin it to start menu from there.
