what is the real meaning of the ' refreshing pc ''?

when I refreshed the pc without affecting the files option from the setting tab and inserted the disc .later I found that only those applications which were installed were removed and the files were unchanged. were those applications creating the problems and they were removed?if yes then what were the problems?
and if the files are left unchanged then what about the virus that those files and folder contain.?
so frens please make me understand the significance of that option and when I need to refresh the pc ?

In brief: Refresh Your PC in Win8—Attempts to solve system problems by replacing system files without affecting user data files. Such files still should be backed up. Installed programs will have to be reinstalled except those from the Windows Store. A list of uninstalled programs is provided on desktop. You personalization settings will be maintained. System settings will be set to defaults. A Windows.old folder will be created at C:. You can delete it once you have everything you want from it--Windows.old Folder - Delete in Windows 8.

Refresh also sometimes in needed in activation: Clean Install with Windows 8 Upgrade.
