Looks pretty dead SD card from here. Can you try it on some other device, mobile phone etc. ? If it was not formatted or wrong format it should show up in disk management at least.
This is a long shot but any chance you're using an older card reader? Older card readers could only handle max 4GB cards. Has the card been readable in the past?
