Solved BSOD: APC_INDEX_MISMATCH during general use

Hi there

I'm getting APC_INDEX_MISMATCH BSODs while using the computer, or away.

Win 8.1 on a Vaio laptop that I've previously disabled the LAN for the docking station (as that seemed to be causing different crashes, but some time ago now, in case that's relevant)

SF debug tool logs attached, thanks for your help.

APC_INDEX_MISMATCH (1)
This is a kernel internal error. The most common reason to see this bugcheck is when a filesystem or a driver has a mismatched number of
calls to disable and re-enable APCs. Basically, the number of times a driver enters into the Kernel Space must leave it exactly that number of times before it exits. This is enforced in order to ensure the security and stability of the system.

Kindly follow the below steps, one step at a time -


  1. Please remove Sony Firmware Extension Parser. It has known issues with Windows. If you are not able to find the software, kindly do a manual system wide search for the driver in RED and then rename it to "SFEPbak.sys".
  2. Kindly remove TrueCrypt till the time we are troubleshooting because it is also known to cause issues.
  3. Please update the drivers highlighted in BLACK, BOLD and UNDERLINE.
  4. If you still face problems, kindly turn on Driver Verifier by following this **GUIDE**. Once the system crashes for a few times, kindly re-run the SF Diagnostic Tool and upload a freshly generated ZIP File.

Since my message, I recieved a different BSOD which may or may not be relevant.

No success in finding updates to drivers so removed them:
SFEP.sys driver renamed, device removed & disabled upon reappearing
HECIx64.sys intel management engine - device disabled, driver renamed
rimssne64.sys memory stick - device disabled, driver renamed

Haven't bothered reinstalling simply uninstalled the following:
tap0901.sys openvpn - uninstalled, driver file deleted
truecrypt.sys - uninstalled, driver file deleted
speedfan.sys - uninstalled, driver file deleted


Hopefully that will solve the issue. I'll try to remember to update you if I don't get any more, but if not assume it's sorted. Thanks again.

If you are still facing issues, kindly re-run the SF Diagnostic Tool (Once you get few BSODs with Driver Verifier) and upload the latest ZIP file in your next post so that it could be analysed :)

Driver verifier ran, new diagnostic tool zip file attached.

After seeing the driver BSOD several times (l1c63x64.sys), which appears to be the Ethernet controller, I remember that in the past I disabled the ethernet port on the docking station I use as it was crashing due to that. It may or may not also be relevant that I had disabled the internal 3G modem as well, not sure why but assume it either wasn't working or causing crashes also.

Anyway, I'll wait to hear what you think!

So, the instruction was to move the 64 bit pointer which is pointing to the r14 register to the r14 register (I know this sounds confusing/unusual and that is why this is wrong). We see that the Qualcomm Ethernet Controller driver doing the stuff which resulted in the crash as per Driver Verifier and the Dump file.

Driver updated. The docking station appears to use the same chipset for the Ethernet, but I'll keep that disabled for the time being till I see if this has sorted it.
