Solved BSOD Error code 139

I've been having problems this month with blue screen error 139, and would like help figuring out what is happening.

Only 160 Windows Updates installed. Most systems have 190 or more. Please visit Windows Update and get ALL available updates (it may take several trips to get them all).

Many, many Live Kernel Events in the WER section of MSINFO32. Please do the following:
- open Event Viewer (run eventvwr.msc from the "Run" dialog))
- expand the Custom Views category (left click on the > next to the words "Custom Views")
- right click on Administrative Events
- select "Save all Events in Custom View as..."
- save the file as Admin.evtx
- zip up the file (right click on it, select "Send to", select "Compressed (zipped) folder")
- upload it with your next post (if it's too big, then upload it to a free file-hosting service and post a link here).

Many, many errors related to the The Sims 4.
Please temporarily uninstall it and see if that helps

Another possibility is a file system or hard drive problem.
Please run these hard drive diagnostics on ALL hard drives: Hard Drive Diagnostic Procedure
Also, please run CHKDSK /R /F on all volumes (reboot may be required when running it on the OS drive).

The chkdsk log shows several corrections being made to the file system.
I'd suggest running it again after you've experienced another BSOD (if that happens)

It could be that a program was interrupted while writing to the disk (by the BSOD)
Or it could be a fault in the way that things write to the disk.
So, if the problems recur, we'll have to look at different possible solutions.

Here's 2 of them (it won't hurt to run them now if you'd like):
First, please run the following DISM commands to see if there's any problems with the system (from an elevated (Run as administrator) Command Prompt). Press Enter after each one:

FYI - I have repaired systems using the last command even though problems weren't found with the first 2 - so I suggest running them all.

Secondly, you can also run sfc.exe /scannow from an elevated (Run as administrator) Command Prompt to check for further corruption.

I went ahead and ran dism (in order of the post) and sfc, and I will run chkdsk, dism, and sfc again if I get another BSOD, along with posting the minidump.

It looks like DISM also repaired some stuff - so I hope that it (and chkdsk) have solved your problems.

Just when I thought I was BSOD free, I receive the same BSOD while looking up prices for a PS4 on Firefox.

Edit: Ran Malwarebytes, and the only thing found in the registry was this: PUP.Optional.WeDownLoadManager.A. I got rid of that.

In msinfo32.nfo > Software Environment > Windows Error Reporting, I found various oddities such as 12/1/2014 at 4:40 pm - Application Error (how could I use a program in the future when the BSOD happened around 2 pm?), 1/1/1970 12:34 am - Application Error (makes no sense), and some other application error times that made sense time-wise, yet made no sense in the sense that I never ran things like TS4 during those times those App errors happened, nor do I have anything set for TS4.exe to auto start up (or Origin). I also noticed in updatelog.txt, for 12/1/2014 at 13:31, 6 updates were detected, and yet when I check Windows Update for updates (about 10-15 times) Windows Update says everything is up to date (I'm wondering because I've had Automatic Updates disabled for a good few months now). Could you explain any of this to me, if its not too much trouble?

Applications run in the background - and there are many of these on the average Windows systems. So it's likely that these applications were running. I've had issues with the time in these reports - they just don't seem to match with the times that are on the BSOS memory dumps - so I have to wonder which are correct and which aren't.

FWIW - what you don't see in MSINFO32 is the followup things that Windows has done. Most of these entries are available in the Event Viewer logfiles. For example, in the Application log file you'll find the errors that you mentioned. But there may be subsequent entries (that are not errors - these may be labelled "Warning" or "Information") that say the problem was solved.

I use the WER section of MSINFO32 (and the Administrative Events in Event Viewer) as a starting point. They give us suggestions on where to look for further details about the problems. In most cases of a BSOD - the application errors aren't significant. BSOD's are a result of problems in the kernel memory space - and applications are limited to the user memory space. This means (basically) that applications can't directly cause BSOD's.

If you've had Windows Updates disabled for months and still don't have any updates available - please post over in the http://www.eightforums.com/windows-updates-activation/ for further expert assistance with this issue.

The DISM may have fixed things, so check to see if there are still problems with the updates.

Let us know the results of the chkdsk.

Just FYI - I'm starting to suspect video (uncertain if it's hardware or software) due to the presence of video drivers in the raw stack text. This isn't a very significant finding, as video is present in most everything that you do with your system - but it does bear watching at this point.

The Windows Update situation brings up another question - why haven't you installed Win8.1?
Is it because of problems with it, because you didn't want it, because it's never been offered, or maybe something else?

[DEL]If this really is a video problem, I want to say this is probably a driver fault. I say this because I use Linux almost as much as Windows on this computer, and not once have I ever had any video problems, or any problems for that matter.

Edit: Scratch that, starting today I've noticed odd graphical problems both in Windows and Linux, mostly screen tearing and weird black boxes when the system is busy doing something (such as at startup, or copying a huge amount of data). Would it be best for me to get my graphics card replaced, since ive only had it for a couple of months?
Edit #2: Linux froze and crashed shortly after first edit, going to have graphics card replaced immediately. Will let you know how that goes.

At the line PROCESS_NAME: MMMon.exe, I know that belongs to my mouse. Would disabling that from computer startup also help any?

On Windows 8.1, one of the main reasons I didn't upgrade to it because I do not want it, for various reasons, one of them being me not seeing any reason to upgrade at this time.
I was always constantly offered to upgrade (until I disabled that), and I did have a few problems when Windows 8.1 first came out (pretty sure it's fixed now).

Before I changed most parts (except for RAM and hard drive), there was no memory reserved for hardware.

Chkdsk looks good to me. No problems found.

The video card problem seems to be getting worse. If you can swap the card, please do so.
If you must purchase a new card, be sure to get it from a place that allows returns (just in case it's not the problem).

The process that was running isn't usually significant - so I wouldn't blame your mouse at this point.
The only time I use the Process name in a diagnosis is if the same process is repeated over and over again.

Don't worry about 8.1 for now. I tried for months to install it until it finally worked for me.
I just wanted to know if it was causing problems for you.

The memory numbers look normal to me. Individual settings changes could account for the differences.
FWIW - my system has the .1 gB difference also - and Hardware reserved of 113 mB

Took out old graphics card, and replaced it with one I had lying around (while I wait for a replacement). So far, no BSODs. I will wait a couple more days before marking as solved.

Unfortunately, while looking up information on firefox about 10 minutes ago, I got the same BSOD again...
I probably should have waited a little longer.
So far, the only thing I ran was sfc /scannow, which found no problems. I will run dism and chkdsk later.

Edit: Ran Dism, /ScanHealth and /CheckHealth found nothing, and /RestoreHealth may or may not have done anything. Im going to attempt to disable NvStreamKms.sys for now.

We'll have to get a bit more aggressive here - as our previous efforts haven't solved your problem.

Have a look at the different driver lists from the memory dumps.
Most common in them are drivers from Virtual Box,from Avast, and from nVidia.
I would suggest:
- switching to another antivirus (temporarily) and uninstalling Avast
- uninstalling Virtual Box (temporarily) until you see if the system crashes again or not.
- updating your nVidia video drivers (to include the NvStreamKms.sys driver)

In short, we'll try to rule out:
- 3rd party drivers, then
- hardware issues, then
- Windows problems

There were symbol problems with this last dump - in particular the symbol server couldn't locate symbols for ntoskrnl.exe - the core of the operating system. Have you done anything to the OS that might affect it? How did you install windows and where did you get the installation media that you used for the installation?

I see that you haven't installed Win8.1 Is there a particular reason for this?

I'll go do those suggestions then. If i've done anything to the OS (even inadvertently), i'm not aware of it.

I installed Windows with a cd that I created from an iso I got from a program called Dreamspark.

Yes, there are a few reasons I have not installed Windows 8.1, and I think its in this thread somewhere. One of the main reasons was I do not see any reason as to why I would need to upgrade, and the other main reason was I had some significant trouble trying to get 8.1 to work properly when it first came out.
