Solved BSOD - Bad Pool Header and Page Fault in Non Paged Area

Hello,

I am very new to the forums and was browsing around looking at other people with similar problems and thought I would post up my issues and see if anyone can help point me in the right directions. I have a couple of ideas but I am unsure if I am right...

I have attached the ZIP of the SF Diagnostic Tool to this post now

Basically about 3 weeks ago I had some computer issues, I figured it was a good time for a couple of upgrades so I replaced the Motherboard, Processor, Power Supply, and I had my Video Card RMA'd by EVGA (excessive coil whine)

I formatted the hard drive (it was a 300GB partition of a 1.5 TB Raid 0) and reinstalled windows 8 to the computer, everything seemed fine for about a week, I took my computer downstairs to hook it up to a TV so my family could watch some Hulu on the big screen and my computer started getting BSOD every few minutes.

I was unsure as to what was causing it so I reinstalled windows again. After the install I was still getting BSOD... so I went out and bought a SATA 120GB SSD and put it in as the primary Hard Drive and installed windows to that drive instead of onto the Raid partition. Same thing, more BSOD...

At this point I have reinstalled 4 times, I tried allowing Windows to fully update everything and tried turning all updates off and manually installing everything myself. More BSOD both ways... in fact at this point when I try to manually install the NVidia drivers they just Fail during the install and wont finish...

Given that the BSOD seem to be either Page Fault in Non Paged area and Bad Pool Header I figured it might be the RAM and ran a MemTest over night, unless I am reading it wrong there were no errors:

My current system specs are:

Full version Windows 8 Pro (not upgraded to 8.1)
Intel i5-4670 CPU (about 3 weeks old)
Gigabyte Z87X-HD3 Motherboard (about 3 weeks old)
2 2GB Corsair 1333 MHz DDR3 RAM (about 5 years old)
EVGA Nvidia GTX 580 3GB video card (just got a RMA replacement about 3 weeks ago)
SoundBlaster XFi PCIE card (About 5 years old)
PCI 4 Drive SATA Raid Controller
120GB SSD Hard Drive (about 1 week old)
4 500 GB Seagate Barracuda single platter Hard drives (SATA, about 3 years old, 1 single and 3 in a Raid 0 Setup)
Corsair TX 650M Power Supply (about 3 weeks old)
DVD RW Drive (SATA, about 5 years old)

So, based on this wall of information, I am suspecting one of 3 things:

1. Might be corruption in the 8GB Thumb drive I am installing my Windows 8 from. ?
2. Might be something with drivers on the computer... after 4 clean reinstalls I don't see how this can be totally it but...
3. Some other hardware on my computer is causing the issue:
- On board sound conflicting with the soundblaster?
- RAID controller?
- Bad GPU / VRAM?

I am going to start trying to figure it out and was hoping someone could give me pointers.

I ran the Diagnostic tool I got from this site and zipped up the file - it is now attached to the OP

I have windows set to gather the dump file but I am unsure what to do with the files at the moment

I'm late for work, but here's what the analysis found. Will post more after work:

Please note that the Silicone Image drivers (most likely from your PCI 4 Drive SATA Raid Controller ) date from 2004 to 2007. These drivers must be updated to versions that are compatible with Win8. If unable to find Win8 compatible version (I wasn't able to find them for my older controller), then you'll have to get a controller that is compatible w/Win8.

Please update these older drivers. Links are included to assist in looking up the source of the drivers. If unable to find an update, please remove (un-install) the program responsible for that driver. DO NOT manually delete/rename the driver as it may make the system unbootable! :

Update:

Last night when I was going into the computer to get the logs I booted up into the BIOS and Turned OFF the on-board Audio controller (previously it was set to Auto, so theoretically it should have turned it off when it detected the SoundBlaster I had)

Booted up to Windows and the computer BSOD as I tried to log in... Rebooted again, this time I got into Windows but I was getting the Hardware detected sound followed almost instantly by the Hardware disconnected sound on an infinite loop... turned out to be the Wireless network adapter I had plugged into the computer (I have a 2 story house and my Meraki is in the Daylight basement... I have Cat5e cable and plugs but have not had the time to run the cable yet to hardwire my internet through the house) - For some reason the Wireless adapter would load, start connecting to my wireless network, then drop and repeat until after 4 or 5 min I got another BSOD.

Rebooted again, same thing, WiFi adapter started its cycle again so I unplugged the the adapter and just plugged it into the ports right above the ones it was previously plugged into... and it pulled right up, connected to the WiFi and stabilized...

My wife used the internet on the computer for about 20 - 30 min posting pics on her Facebook then I left the computer on overnight again to see if it would BSOD again... and it was still running this morning when I got up. I ran the NVidia drivers that previously had been failing everytime again to see what it would do and they installed with no problems this time.

So, I left the computer on when I left for work and will check on it when I get home. Regardless I bought 2 4GB Corsair Vengeance 2133GHz RAM sticks that I will install tonight (and take out the ones I have in there now) and I will start dismantling the RAID set-up and take it out of the computer if I cant find new drivers.

I do have 1 more question I could use some help with in regards to the RAID, in order to dismantle it I need to move some files off it and the HD I need to move them to has some Old windows folders on it I cant seem to delete - I have already tried taking ownership of them and it still wont let me. I was thinking about trying to build a Boot USB that will go to a command prompt and read NTFS drives (is that possible?) or is there a better way to kill that folder?

You have 1 Windows Update installed. Most systems have 160 or more. Please visit Windows Update and get ALL available updates (it may take several trips to get them all).

You have an EnGenius 802.11n Wireless USB Adapter:
I do not recommend using wireless USB network devices. Especially in Win7/Win8/8.1 systems.
These wireless USB devices have many issues with Win7 and later - using Vista drivers with them is almost sure to cause a BSOD.
Should you want to keep using these devices, be sure to have Win7/Win8/8.1 drivers - DO NOT use Vista drivers!!!
An installable wireless PCI/PCIe card that's plugged into your motherboard is much more robust, reliable, and powerful.

Is there a particular reason that you're not using UEFI or Secure Boot?

You have 1 Windows Update installed. Most systems have 160 or more. Please visit Windows Update and get ALL available updates (it may take several trips to get them all).

There is only 1 installed because that is the one that came on the Motherboard Driver DVD, I had turned them off after my 4th reinstall because it seemed that the updates just caused the BSOD to get worse. I will go ahead and allow it to start updating again.

You have an EnGenius 802.11n Wireless USB Adapter:
I do not recommend using wireless USB network devices. Especially in Win7/Win8/8.1 systems.
These wireless USB devices have many issues with Win7 and later - using Vista drivers with them is almost sure to cause a BSOD.
Should you want to keep using these devices, be sure to have Win7/Win8/8.1 drivers - DO NOT use Vista drivers!!!
An installable wireless PCI/PCIe card that's plugged into your motherboard is much more robust, reliable, and powerful.

It was supposed to be a temporary stop-gap until I could get my house wired for Cat5e, I really should just go get a Wireless card as the wiring project got put on hold when my father passed away.

Is there a particular reason that you're not using UEFI or Secure Boot?

To be honest, I just never have. I will do some research into it, but any pointers/advice you can give on them would be appreciated.

OK, my computer didn't BSOD from yesterday evening until I got home today and turned the Windows Updates back on, since turning it on I have gotten TONS of BSOD... not sure why that made such a huge difference, but here are the new logs I pulled tonight - I did remove the ON/OFF driver, update the USB WiFi adapter driver to the newest win 7 version, and update all the RAID controller drivers to the latest Win 7 version.

I cant seem to delete a Windows folder off my backup drive from a really old Windows install, once I can get it deleted I am planning to move everything off my RAID and just dismantle it and remove the card from the system.

Hopefully I can replace the USB WiFi with a WiFi card and put the RAM in tomorrow night.

Anything anyone can tell me about why the Windows updates seems to cause crashes or if that is just coincidence would help.

There are many different reasons for BSOD's. In many cases of BSOD's associated with Windows Updates - it's because some 3rd party (non-Windows) program isn't compatible with the updates. Also, on a system that has 160 Windows Updates - it only takes 1 missing update to cause problems - what are the chances for problems if that same system only has 5 updates (as yours does)?

Now it appears that you only have 5 Windows Updates. Please keep visiting Windows Update until you've had several visits that didn't need Windows Updates (and these were after rebooting the system). This is going to take a while - it often takes several hours when we do this at work (and we have customized programs that do most of it for us).

The old Silicone Image drivers are still present. I suspect that they are the main cause of the BSOD's. As such you've either got to get newer drivers, or uninstall the software for this device and physically remove the device from your system. Please provide the link that you used to update to the Win7 drivers (that way we may be able to see the exact versions in the download).
If the manufacturer doesn't offer Win8 drivers, then the device isn't compatible w/Win8
If you are able to get Win7 drivers to work - they may still have problems.

The same goes for the WiFi - if the manufacturer doesn't offer Win8 drivers, then the device isn't compatible w/Win8
If you are able to get Win7 drivers to work - they may still have problems.

Question about removing the old drivers when I pull the cards:

When I Uninstall a device from my computer I have been pulling up Device manager (unless it is something like the video drivers that install through a program) and just clicking the device and clicking Uninstall to remove it.

Is that the right way to do it or is there are better / more complete way to get rid of them?

You should first try to remove anything through Control Panel...Programs and Features
After that, then look in Device Manager and uninstall anything that's there (related to the device you're removing)
In general Windows is smart enough to cope with unexpected removals - but it doesn't hurt to spend a little extra time with it.

Even then, it may not remove everything. Some things to consider:
- did the installer log everything that it did? I used to use a program that logged all the installations - but that got tedious. Revo Uninstaller will do something similar.
- does the uninstaller remove everything? This includes license information.
- Just yesterday I found a registry entry that was trying to load a driver that had already been removed (a highlighting program). I spent 30 minutes researching it only to find out that it was a remnant in the registry and that no driver was present (and none of the registry cleaners that I tried even spotted it).

FYI - I only use registry cleaners that allow me to choose what to remove. Letting a program do it is (IMO) a recipe for disaster!

So I put the new RAM into the computer, uninstalled the programs and drivers for the old Wifi adapter and pulled it then replaced it with another USB Wifi adapter that has a Windows 8 driver - and removed the Sound Blaster card.

When the computer came back up I was able get windows to install all but 2 of the updates it had available, one of the updates was to the drivers for the RAID controller - so I have not pulled that card from the computer yet. (trying to back up the data)

The 2 updates that will not install are both for .net 4.5 and they both fail even when I download the standalone installers from Microsoft and try them manually, but everything else worked.

But, so far the computer has been running stably since Saturday morning even through all the windows updates and some Graphics / CPU stress tests.

Tentatively I think the problem is resolved. I will give it a couple more days before I mark this post as solved, thanks!
