Multiple BSOD - MEM_MGMT, SYS_SVC_EXC, PFN_LIST_CORRUPT

Good afternoon,

I have been having trouble with a custom build PC running Windows 8.1 Pro since I first powered it up (I didn't build it myself). Crashes most often occur when browsing using either IE 11 or Google Chrome 31, or when attempting to transfer files from external hard drive via USB; however the system has also crashed during other operations and when idling.

BSOD error codes include:

DPC_WATCHDOG_VIOLATION
KMODE_EXCEPTION_NOT_HANDLED
IRQ_LESS_THAN_OR_NOT_EQUAL
APC_INDEX_MISMATCH
ATTEMPTED_EXECUTE_OF_NOEXECUTE_MEMORY
MEMORY_MANAGEMENT
SYSTEM_SERVICE_EXCEPTION

FILE_SYSTEM
ATTEMPTED_TO_WRITE_TO_READONLY_MEMORY
PFN_LIST_CORRUPT

The last six of these (in italics) have occurred today, after all the fixes below have been attempted.

Fixes applied:

Reinstalled Windows
Updated BIOS
Updated Chipset drivers
Updated SSD Firmware
Updated all drivers I could find to update
Memtest86+ (5 passes, no errors found)
SFC /scannow (no errors found)

I have attempted Automatic Repair a couple of times with no success. Attempted System Restore with installation boot disk earlier today and received the following error message:

"There was an unexpected error: Configuration registry database is corrupt (0x800703F1). Please close System Restore and try again."

I have attached dump files from 12/19 and today, and am running Driver Verifier now per instructions. System specs are in my profile below.

Update: For the third time today, screen is frozen, but still able to move cursor with mouse. No response to clicking, however. First two times, happened on Start screen immediately after login. This time it happened after closing browser, then moused over to reveal Charms bar. Screen froze with Charms bar open.

Sorry for the delay in responding. There just aren't that many people who do BSOD analysis, so at times we get a bit overwhelmed!

Your second upload (SF_21-12-2013.zip) is corrupted, please upload it again.

You're describing a hardware problem here (although SSD firmware issues can do this also). Repeated BSOD's w/different error codes are typically hardware or low-level driver issues. Start with these free hardware diagnostics: Hardware Diagnostics

Just a hunch, but check to make sure your video card is cooling properly (fan is working, cooler is clear of dirt/dust, cooler is getting cool air) Monitor the temps with this free utility: HWMonitor CPUID - System & hardware benchmark, monitoring, reporting

SSD's were temperamental when they first came out, but their reliability has improved greatly over the last year or two. There are 2 things that MUST be checked on systems with BSOD's and SSD's:
1) That the SSD firmware is the latest available for the SSD. I usually can't see this info in the reports, so I rely on you to check it.
2) That the storage controller drivers on the motherboard are the most recent. They must date from late 2012 or later. This is ALL controllers, not just the one that the SSD is attached to. If involved these should be visible in the memory dump - but it doesn't hurt to check manually.

Is there any particular reason that you are not using UEFI/SecureBoot? It is my experience that Win8/8.1 systems are generally more stable when using UEFI/SecureBoot.

Please uninstall the Western Digital software. The wdcsam64.sys driver dates from 2008 and is known to cause BSOD's on some Windows systems.

Please note the drivers below that date from before the release of Windows 8.1 (17 Oct 2013) - check to see that each and every one is compatible w/Windows 8.1 (at the manufacturer's website). If not, you must either remove the device and it's software - or locate a Windows 8.1 compatible update on the web.

If all of this doesn't stop the BSOD's, please run these free hardware diagnostics: Hardware Diagnostics. Let us know the results and we can move on from there.

  2. SSD firmware: Using Samsung Magician, updated to EXT0BB6Q, released 12/2013.
  3. Storage controllers: Only thing showing in Device Manager under Storage Controllers is "Microsoft Storage Spaces Controller". Are there other storage controllers somewhere I need to update?
  4. UEFI boot: I thought I was using Secure Boot. BIOS seems to say so, but I'm no pro at this. I have attached screenshots from the BIOS. If it's not setup correctly, how would I do so?
  5. WD software: I've tried uninstalling the WD SES driver in Device Manager, but every time I plug the external drive into the USB port, the driver shows again in Device Manager. There are no WD programs showing in the Programs list in Control Panel.

Storage Controllers - no need to worry about them as they'll show up in the dump files if they are involved - and they didn't show up in the previous dump files.

UEFI Boot - I'm not an expert at this, but the screen shots seem to indicate you are using SecureBoot - yet the MSINFO32 file says you're not using it. If you wish to pursue this, I'd suggest posting over at the Asus forums for clarification.

The WD software isn't needed - so you can stop it from loading. I'd suggest first making a recovery drive and a System Restore point (just in case) - then stop the wdcsam64.sys driver from loading. Here's the detailed instructions:

1) Create a Restore Point using System Restore
2) Create a Repair disc (Recovery Drive in Win8.1):
Win 7 - Go to Start...All Programs...Maintenance...Create a System Repair Disc
Win 8 - Press "WIN" and "R" to open the Run dialog...type "RECDISC" (without the quotes) and press ENTER
Win 8.1 - Go to the Start Screen and type in "recoverydrive" (one word, without the quotes). That will start the recovery drive process. You will need a USB drive of at least 512 mB - and all data will be erased off of it.
3) Test the System Repair disc/Recovery Drive to make sure that you can get to the System Restore entry when you boot from the disk/drive (you may also want to try actually using System Restore to make sure that it works)
4) Download this free program (Autoruns for Windows) and use it to disable any instances of wdcsam64.sys that are starting (or any other Western Digital entries) (DO NOT DELETE - only disable by removing the checkmark in the left hand column)
5) Check in Device Manager (to include showing hidden devices from the View menu item) and ensure that any instances of wdcsam64.sys (or any other Western Digital entries) are "Uninstalled" (DO NOT DISABLE THESE).
6) Check in the Services applet (services.msc) to be sure any instances of wdcsam64.sys or any other Western Digital entries are disabled.
7) Go to C:\Windows\System32\drivers and rename the wdcsam64.sys driver to wdcsam64.BAD (search the hard drive for it if it's not in C:\Windows\System32\drivers).


In the event that the system doesn't boot:

1) Boot from the System Repair disc/Recovery Drive and use the Command Prompt option to rename wdcsam64.BAD to wdcsam64.sys

2) Boot from the System Repair disc/Recovery Drive and use the System Restore option to restore the system to a point before the changes were made.

Please note the drivers below that date from before the release of Windows 8.1 (17 Oct 2013) - check to see that each and every one is compatible w/Windows 8.1 (at the manufacturer's website). If not, you must either remove the device and it's software - or locate a Windows 8.1 compatible update on the web. Don't worry about these 2 drivers: intelppm.sys and dump_storahci.sys

Once the Western Digital driver is removed and the rest of the drivers are verified as Win8.1 compatible - if the BSOD's continue, then a hardware problem is the most likely explanation (IMO).
This is evidenced by the numerous different BSOD error codes. I would suggest finishing up on the hardware diagnostics and letting us know what the results are.

In among the rest of the memory dumps are a few dumps pointing to the video drivers. These dumps are listed as Driver Verifier Enabled. Are you running Driver Verifier? If so, please make sure that it's turned off (if not, then don't worry about it as these dumps can occur without Driver Verifier running). I still suspect the video card, so the video stress tests should be run ASAP.
