IRQL_NOT_LESS_OR_EQUAL and APC_INDEX_MISMATCH

Hello!
I have an ongoing problem with BSOD-s. Now I had 2, with only ntoskrnl.exe being the problem according to BlueScreenViewer, with the messages mentioned in the title. I included the .zip file made with SF diagnostic tool. Please help me in where can I find the problem.

I think I deleted the drivers, but now I got a BSOD with the following message: SYSTEM_SERVICE_EXCEPTION. I attached the files made with the diagnosis tool. Hope you can help me resolve the problem.

Out of 4 memory dumps there were 4 different BSOD error codes. The differing error codes are usually symptomatic of a lower level problem within the system. They are usually caused by one of these things:
- borked (broken) hardware (several different procedures used to isolate the problem device)
- BIOS issues (check for updates at the motherboard manufacturer's website)
- overclocking/overheating - You'll know if you're overclocking or not. If uncertain we can suggest things to check.
- compatibility issues (3rd party hardware/drivers)
- low-level driver problems
- or even malware (scanned for when we ask for hardware diagnostics from http://www.carrona.org/initdiag.html or http://www.carrona.org/hwdiag.html ).

FYI - I have seen 1 or 2 issues with the wireless USB XBox controller. The previous version was known BSOD cause - but this one seems a bit more stable. Regardless, I'd suggest uninstalling it's software and physically removing the device from your system in order to test it. If it doesn't stop the BSOD's, feel free to reinstall it after the system has been stabilized.

I would first completely remove your video drivers, then install a freshly downloaded copy of the latest, Win8 compatible drivers for your video card.

If that doesn't fix things, then I'd try the video stress tests from the hardware diagnostics that I posted earlier ( Hardware Diagnostics).

Hello!
I've tried reinstalling my display driver multiple times, but with no success (as seen above). Could you please tell me exactly which driver should I download and how to actually delete the previous one completely? (I'm starting to doubt that I do it right, since it doesn't help at all.) I have a Sapphire HD 7770 GHz Edition. I hope it's not that big of a setback or problem, would help immensely.

I did everything, and now got a BSOD again, I include the files needed and also included the BlueScreenViewer file in case it is needed.
EDIT: just got another one with BlueScreenViewer saying that nothing went wrong, still bsod. I'm clueless.

I ran memtest, hddiag and furmark. Neither of them showed any errors. I printscreened the state which furmark stabilized in. I ran it 3 times for about half an hour and every time it was exactly the same.

I ran the video tests (vmt and memtestCL). MemtestCL showed no errors, as well as vmt twice, but one time it showed something different, I include the result file. I ran the intel diagnostic tool, it showed no errors too, I ran Prime95 today, everything went fine until about 4 hours of runtime, when the computer crashed with the IRQL_NOT_LESS_OR_EQUAL, not sure if it was because Prime95 or because I had to use the computer for a while. (I ran it in Small FFTs, since my comp already passed memtest86, although I can run it in blend overnight, but I can't monitor it that way).
EDIT: I got a new one, might be interesting for you, I don't know.
EDIT2: Seeing from what is happening, the computer crashes most frequently when I open World of Warcraft, it is just a wild guess, but that's what I observed from the past 2-3 days.

As you've updated your video drivers and the only signs of issues are video, I'd have to suggest trying another video card.
If you don't have one on hand to try, then I'd purchase one that you can return (in case it's not needed).

Don't forget to remove the drivers for the old card, and install fresh, new drivers that are compatible with the OS.

Hello!
I changed my graphics card to my older one, (Sapphire Radeon HD3850) and once again I got BSOD-s. I included the files necessary. I've used that card for a good 2 year and it worked perfectly.

All 3 memory dumps say "hardware". If we presume that the old video card is good, then the only thing left is the motherboard.
Do you have other slots that you can try for a graphics card? If not, then we're looking at trying a new motherboard (this presumes that you've done all of the hardware tests - to include the CPU stress tests).

I did notice that the XBox controller is still installed. While not a likely cause at this point, we must rule out all possibilities before coming to the conclusion that it's the motherboard (and then going through the cost and pain of installing another motherboard).
